Professional blood draws performed in the comfort of your home, workplace, or facility. We partner with trusted laboratories to ensure accurate, timely results.

Additional Information

Professional mobile phlebotomy at your home, office, or hotel. Base rates include travel within a 30-mile radius of Chambersburg, PA.

· Standard Mobile Draw: $100 Professional collection and delivery to local LabCorp/Quest/Hospital/Private Labs

· Fasting/Early Morning Draw (5:00 AM – 8:00 AM): $150 Prioritized scheduling for fasting requirements.

· Emergency / Same-Day Service: $190 Urgent collection requests with 2-4 hour response time.

· Mileage Fee: $1.00 per mile for locations beyond 25 miles.

Membership Tiers
Members may select one of the following membership tiers:

Tier 1 – Monthly Maintenance Package: $95 per month. Includes one (1) scheduled mobile blood draw visit per month. Recommended for maintenance labs, vitamin checks, and stable chronic conditions.

Tier 2 – Weekly Wellness Package: $340 per month. Includes up to four (4) scheduled mobile blood draw visits per month (once per week). Recommended for preventative and wellness labs, family-requested oversight, and chronic conditions.

Tier 3 – Advanced Wellness Package: $750 per month. Includes up to ten (10) visits per month (Monday through Friday). Recommended for intensive short-term monitoring, clinical trials, A1C monitoring, kidney function testing, and medication titration.

Tier 4 – Daily Care Premium Package: $1,500 per month. Includes up to twenty (20) draws per month (Monday through Friday). Recommended for oncology monitoring, weekly CBCs, tumor marker labs, and medication level adjustments.

Our mobile diagnostic services bring high-quality medical testing directly to patients—eliminating the need for travel, long wait times, or facility-based appointments. We partner with healthcare providers, long-term care facilities, employers, and home health agencies to deliver accurate, timely diagnostics in the comfort of a patient’s home or care setting.

Terms & Conditions

1. Payment: Payment is due at the time of service. We accept cash, credit cards, debit cards, and approved invoicing for contracted facilities.

2. Insurance: CTU Mobile Diagnostics LLC does not bill insurance directly. Clients will receive an itemized receipt (superbill) for self-submission to their insurance provider if desired.

3. Lab Processing Fees: Prices listed above are for specimen collection and rapid testing services only. Laboratory processing fees charged by reference laboratories (LabCorp, Quest, etc.) are separate and billed directly by those laboratories or through the ordering physician.

4. Cancellations: Please provide at least 2 hours notice to cancel or reschedule your appointment. Cancellations with less than 2 hours notice or no-shows are subject to a $35.00 fee.

5. Orders Required: All non-rapid-test laboratory blood draws require a valid physician order. CTU Mobile Diagnostics LLC does not provide diagnosis or medical advice.

6. Service Area: We proudly serve South Central and Central Pennsylvania. Travel fees are calculated from our base location.

7. Pricing Subject to Change: Prices are subject to change without notice. Please confirm current pricing at the time of scheduling.

HIPAA Compliance Statement

CTU Mobile Diagnostics is committed to protecting the privacy and security of all patient health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). We maintain strict administrative, technical, and physical safeguards to ensure the confidentiality, integrity, and availability of Protected Health Information (PHI). We collect, use, and disclose PHI only as permitted by law and solely for purposes of treatment, payment, healthcare operations, or as otherwise authorized by the patient. All CTU Wellness Specialists and staff members are trained in HIPAA compliance and follow standardized procedures to prevent unauthorized access or disclosure. CTU Mobile Diagnostics implements secure data handling practices, encrypted communication methods, and controlled access to patient records. We continuously review and update our policies to remain compliant with federal regulations and industry best practices. Patients have the right to access their health information, request corrections, and obtain details about how their information is used or shared. MEMBERSHIP TERMS OF SERVICE

CTU Mobile Diagnostics LLC

These Membership Terms of Service (“Terms”) govern the purchase and use of membership services provided by CTU Mobile Diagnostics LLC (“CTU Mobile Diagnostics LLC,” “CTU,” “we,” “us,” or “our”). By enrolling in a membership and checking a box or clicking “I Agree,” you (“Member” or “you”) acknowledge that you have read, understood, and agree to be bound by these Terms.

Membership Services
CTU Mobile Diagnostics LLC provides recurring, scheduled mobile blood draw, mobile phlebotomy, rapid testing, and specimen collection services at a Member’s residence, office, or other approved location within our designated service area in South Central and Central Pennsylvania. Membership services provide scheduled visits according to the tier selected by the Member.

All visits must be used within the applicable monthly billing cycle. Unused visits do not carry over.

Scheduling and Appointment Windows
Appointments are scheduled within a two (2) hour arrival window. Requests for a specific, non-windowed appointment time are subject to a $25 additional fee.

No-Show Policy
If the Member is not present at the scheduled location during the assigned arrival window, the visit shall be considered forfeited. Forfeited visits will not be rescheduled or refunded and will count toward the Member’s monthly allotment.

Billing and Auto-Pay
Enrollment in membership services requires automatic monthly billing (“Auto-Pay”). The initial charge will occur on the date of enrollment. Thereafter, membership fees will be automatically billed on the same calendar date each month. Members are responsible for maintaining a valid payment method on file at all times.

Cancellation
Memberships operate on a month-to-month basis. Members may cancel by providing at least fifteen (15) days’ notice prior to the next billing date. Cancellation requests received fewer than fifteen (15) days before the next billing date will apply to the subsequent billing cycle. No partial month refunds will be issued.

Service Area
CTU Mobile Diagnostics LLC’s standard service area includes locations within thirty (30) miles of our base of operations. Service requests outside this radius may incur an additional travel fee, which will be communicated to the Member in advance.

Member Responsibilities
The Member agrees to be present and available during the scheduled arrival window, provide accurate and complete information as requested, follow all pre-draw instructions including fasting requirements, maintain a valid payment method for Auto-Pay, and notify CTU of any changes to contact information or service address.

CTU Responsibilities
CTU Mobile Diagnostics LLC agrees to send qualified personnel to perform specimen collections, handle and transport specimens in accordance with applicable health and safety requirements, deliver specimens to the designated laboratory in a timely manner, maintain confidentiality in accordance with HIPAA, and communicate scheduling changes or service-related issues promptly.

HIPAA Compliance and Confidentiality
CTU Mobile Diagnostics LLC handles Member health information in compliance with HIPAA and all applicable state and federal privacy laws. Member information will not be shared with third parties except as necessary to perform membership services or as required by law.

Limitations of Responsibility and Liability
CTU Mobile Diagnostics LLC is responsible solely for the proper collection and transport of specimens. CTU is not responsible for laboratory processing, testing, results, or errors occurring after specimens are delivered to the laboratory; medical diagnoses, treatment decisions, or clinician orders; or delays caused by laboratories, couriers, weather, or circumstances beyond CTU’s control. CTU’s total liability under these Terms shall not exceed the amount paid by the Member for the applicable monthly membership term.

Non-Transferability
Membership services are provided exclusively to the registered Member and may not be shared, assigned, or transferred to another individual.

Modifications to Terms
CTU Mobile Diagnostics LLC reserves the right to update or modify these Terms at any time for publication on its website. Updated Terms shall apply to future billing cycles.

Governing Law
These Terms shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania.
